Mentee - Background & Screening

Thank you for expressing your interest in being mentored with the Anchor Mentoring ProjectThis is a two-stage process.

Stage 01:
Basic Info & Background Checks

In this first stage, we’re asking for some basic information about you, including your Working with Children Check and Criminal Record Check, and we’ll also invite you to complete a brief screening assessment. This is a standard part of our processes to ensure the wellbeing of everyone involved.

We’re not collecting information about your experience just yet, we’re simply confirming that all foundational requirements are in place and learning a little about who you are.

Stage 02:
Brancher Mentoring Platform Onboarding 


If this stage is successful, you’ll be invited to create a secure user profile in Brancher, our online mentoring platform. There, we’ll collect more detail about your background, your availability, and your interests to help us make the best match possible.

About you


Depression Anxiety Stress Scale 21

Before you get started in the Anchor Mentoring Project, we ask you to complete a short questionnaire called the DASS-21. That’s the Depression, Anxiety, and Stress Scale. It’s just a simple tool that helps us make sure you’ve got the right supports in place before you begin.

It only asks about how you’ve felt over the past week—so it’s not a diagnosis, and it absolutely won’t stop you from being part of the program.

Your responses are kept strictly confidential and stored securely using Snapforms, which is an Australian-based platform that follows local privacy laws.

This is just one small way we care for the people in our community—because mentoring is about connection, not perfection


Please read each statement and select which indicates how much the statement applied to you over the past week.  There are no right or wrong answers.  Do not spend too much time on any statement.


The rating scale is as follows:

0   Did not apply to me at all (Never)

1   Applied to me to some degree, or some of the time (Sometimes)

2  Applied to me to a considerable degree, or a good part of time (Often)

3   Applied to me very much, or most of the time (Almost Always)
